Little Fish Tail Capsular Tension Ring (CTR) Insertion
Thomas A. Oetting, MS, MD
In this case, a capsular tension ring (CTR) was placed in anticipation of possible Descemet's Stripping Automated Endothelial Keratoplasty (DSAEK) following cataract surgery. Our cornea surgeons here at the University of Iowa feel that leaving the patient -1D or so and placing a capsular tension ring makes future DSAEK easier. Rather than use an inserter as I usually do I used the fish tail technique that was described by Agunawela and Little in JCRS . This allows for much less tension on the zonules as the ring is inserted. The ring is bent a bit as it is placed through the 2.75mm wound, but it seems to snap back into position nicely.
