Q20: What is the significance of the corneal findings in this patient with a history of a penetrating keratoplasty?

A20: The white line is a Khodadoust line which is an indicator of endothelial rejection. This "rejection line" is made of white blood cells on the endothelium and usually begins in one region of the cornea and progresses across the endothelium over time. The Khodadoust line in this patient began inferonasally and has migrated to the superior cornea. The region inferior to the Khodadoust line is more edematous and hazy than the cornea superior to the Khodadoust line.
