Donut Hole or Not

Contributor: Tom Oetting, MS, MD; Ed Hu, MD, PhD

In this video, we had trouble with the wound with a posteriorly placed section of the tunnel on the right which allowed fluid running out of the anterior chamber to collect under the conjunctiva causing the dreaded conjunctiva donut or balloon. This is caused often in right handed surgeons by tilting the keratome which makes the right side of the blade lower than the left resulting in the right side of the wound being more posterior. In this case more posterior than the insertion of the conjunctiva.
