Black/Dense Lens Convert To Extra Capsular Cataract Extraction
This patient had Retinopathy of Prematurity (ROP) and developed a very dense cataract over the years. We tried to phaco the dense nucleus but made little progress. His loose zonlules (in my experience very common with ROP) gave me concern that I was going to drop his lens. So we converted to extra capsular cataract extraction. We had planned ahead for this some by making a short limus based incision and with a peritomy. His rhexis was continuous and so i made 2 relaxing incisions prior to expressing the lens. After removing the lens which was essentially black we were relieved to find a red reflex. I did ponder the Intraocular Lens location a bit and with the loose zonules and a capsule tear so we decided to place an anterior chamber intraocular lens (AC IOL). He already had a nice laser peripheral iridotomy (LPI) which was patent. No vitreous came forward during the procedure.
