Complex Rhexis (Little plaque rhexis)

This video shows an interesting rhexis in a patient with an dense anterior subcapsular plaque that was adherent to the anterior capsule. It was a challenge working around this dense plaque. In one area it began to go out a bit and she demonstrated a Little technique to bring it back. frequent addition of OVD (ophthalmic viscosurgical device) will help to keep the anterior face of the capsule flat which makes it less likely to go out. Trypan blue was also used to help demonstrate the anterior capsule in this mature cataract.
