Cow Hitch Scleral Suture

This patient was in his 90s and had little vision in this eye from macular degeneration. His IOL was decentered inferiorly and one haptic was in the the anterior chamber caught on the iris. He had had a sector iridectomy and was missing superior iris and had PAS which made placing an anterior chamber or iris sutured IOL tricky. He had a decent capsule in the inferior sulcus so we decided to secure the IOL with a single suture to the sclera across from the existing capsule.
