According to Yanoff and Fine in their Ocular Pathology an iris freckle is caused by increased pigmentation of anterior border layer melanocytes without increased number of melanocytes. An iris nevus results from an increased number of atypical but benign-appearing melanocytes, i.e. nevus cells with variable pigmentation. The darker, larger lesion at 5 o'clock that extends from the pupil margin out over the pars pupillaris appears to be a nevus.
